The Howard - Suamico School District requires verification of birth date and proof of residency for each child registering in our schools. It is illegal to photocopy a birth certificate in Wisconsin . Therefore, when requiring proof of birth we are not allowed to receive or keep on file a scanned, emailed or photo copied birth record.  Please bring each child's birth certificate to the school for the secretary to verify birth date. Proof of Residency is verified by showing documentation showing resident address i.e. Property tax statement, Utilities statement with address etc. All other student registration forms may be mailed, emailed or faxed to the school.
